I will not be able to access any information in my closed account, including but not limited to: submission metadata, sales history, and tax documents.
I understand that I can cancel distribution or other services related to my releases but keep my CD Baby account active, including valid tax and payment information, to avoid waiving any earnings.
I have checked if I have been paid in this calendar year, and if so, have updated my Tax Form delivery method to Postal, and confirmed that the correct mailing address is on file. I understand that CD Baby will post my tax documents to the address on file at the time of account closure.
In the event that CD Baby receives funds related to my releases from any party after the date on this account closure form, I waive my rights to these earnings. I understand that CD Baby could receive funds related to my releases long after I halt services.
